This game have been made on the basis of what happened after the game "Trauma Center Under the Knife". As Derek and Angie goes to Costigar, they are called back by Caduceus USA. They also meet some new persons. As the GUILT returns, their fight begins again after Delphi for stopping these GUILT researches once and forever. That's what the game is for!

Trauma Center Under the Knife 2 continues where Trauma Center under the Knife last left off. It continues after Derek and Angie's war with GUILT created by the organization Delphi led by "Adam".Soon after Derek and Angie their travel to Costigar they are called back by Caduceus headquarters, Derek’s medical organization, to investigate a new GUILT-related disease tentatively called Post-GUILT Syndrome. Afterwards, Delphi begins begins releasing new strains of GUILT. Derek and Angie set out to find the source of these new strains and put a stop to GUILT once and for all!
